Is the Agency licensed?

Yes. We are licensed by Illinois Department of Public Health and also a provider for the Veterans.

How does the Agency select and train its employees? Does it protect its workers with written personal policies, benefits packages and malpractice insurance?

Is someone from the Agency will do the assessment to evaluate the patient Home Care needs? 

Yes. That would be the initial step so we could provide the best match of caregiver to work with your love one.

Is the patient's course of treatment documented, detailing the specific tasks to be carried out by each professional caregiver?

Yes. We will provide a Plan of Care to be coordinated with the family and caregiver. If changes occur it will be updated, and caregiver will document the daily task and if changes occur we will notify the family.

Does the Agency assign supervisors to oversee the quality of care patient's are receiving in their homes?

Yes. Our supervisor will visit every 90 days which is a requirement by the Illinois Department of Health or as needed if the clients have any concerns.

Does the Agency take time to educate family members on the type of care that is being provided?

Yes. That will be the initial responsibility of the Agency and when there are changes to the care of the client.

What Agency's financial procedures?

Agency will provide a initial agreement to be sign by the client and the Power of Attorney of the client. It will explain the costs and responsibilities of the agency associated with Home Care.

What procedures are in place to handle emergencies?

We are available 24 hours a day, 7 days a week.

Does the Agency provide literature explaining its services, eligibility requirements, fees and funding sources? An annual report can provide helpful information about the agency.

Yes. Our Agency will supply clients with a detailed information that will outlines the rights and responsibilities of the providers, clients and caregivers alike.
